Stórhöfði, Eldfell and the boat show different sides of Heimaey

Leave a little room to wait; you do not need to fill every minute with another stop.
A boat tour changes the subject. From the water, the sea caves, rock columns, Elephant Rock and the shape of the archipelago become the main view, with puffins as one part of it. That can suit someone who would rather not hike, or a day that already includes the climb up Eldfell.
Puffin Express solves the distance, not the uncertainty of wildlife. Public product information describes a 40–45-minute transfer to Stórhöfði; once there, you still walk and wait on a windy headland. A shuttle removes the long approach, but it does not turn the viewing area into an instant sighting.

Heading south from the harbour looks like one straight island line, but it is energy you must recover before the return ferry. If you walk, treat the south as the farthest point of the day; if you take the shuttle, give the saved time to the volcano or an indoor attraction near the harbour.
Work back from the ferry, not forward from the puffins
For an early ferry arrival, make the first decision at the harbour: are puffins the main event, or is the boat tour? If you have booked a fixed departure, find the meeting point on the map first, then plan breakfast in town. Ferry delays, the queue after disembarking and luggage storage all reduce the spare time you thought you had.
For land-based puffin watching, the harbour—south tip—town—volcano order works only when the wind is comfortable, your pace is reliable and the return ferry is not too early. After the south, do not add another far-flung coastal point on impulse; the way back can be used for a harbour café or Eldheimar, both close enough to finish on time.
With Puffin Express, the 90 minutes saved are best spent on a genuinely different theme. For example, take the shuttle to Stórhöfði in the morning and climb Eldfell after lunch, or complete the outdoor puffin stop before bad weather and use Eldheimar as the indoor fallback. A short drive does not make two volcanic slopes and another coastal walk fit; your body still pays for the extra plan.
Choose a boat tour on a day when you are happy for the sailing itself to be the main attraction. Anyone wanting sea caves, bird cliffs and Elephant Rock should allow time for windproof waterproof clothing, walking before and after boarding, and seasickness. If a companion is very uncomfortable in waves, a land-based Puffin Hike may produce a more complete afternoon. A changed departure time will not make the return ferry wait.
Puffin season needs a fallback outside the main dates
Island activity pages commonly list Puffin Express and Puffin Hike from May to September, while puffins remain wild animals. In August and September you may also encounter fledglings leaving the nest; Westman Islands activities involving rescued fledglings have a different atmosphere from ordinary birdwatching. If fledglings are your specific reason for visiting, check the activity date, late ferry and accommodation together rather than chasing an evening-only story on a day ferry.
In April or October, the volcano and coast can still make a good day, but the puffin theme cannot simply follow the high-season timetable. I would make Eldheimar and a town walk the main plan, with a boat activity added only if it is operating. The island then remains a day out even if the suitable sailing is unavailable.
Some travellers feel sick as soon as a boat leaves and have no appetite for another sea trip by the time they reach Heimaey; others reach the south and find the wind harder than expected. This is not a test of courage. It is a question of whether the activity suits your body that day. Choose land if you want a steadier bird-watching setup; put the sea trip on the payment list when the island’s terrain is what you want to see in one sweep.
